WEBSITE NOTICE

Regulatory basis: 45 C.F.R. § 149.610(b)(1)(iii) requires that notice of the right to a Good Faith Estimate be written in clear and understandable language and prominently displayed on our website, in our office, and wherever scheduling or cost discussions occur.

Your Right to a Good Faith Estimate

You have the right to receive a Good Faith Estimate explaining how much your care will cost.

Under federal law, health care providers must give patients who are uninsured, or who are not using insurance, an estimate of the expected charges for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs such as medical tests, prescription drugs, equipment, and facility fees.

  • If you request a Good Faith Estimate before scheduling, we will provide it to you in writing within 3 business days of your request. You can also ask any health care provider for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.

Shōka Health primarily operates on a self-pay basis. Paying through a financing service does not change your right to an estimate. Current pricing for ketamine infusion therapy and stellate ganglion blocks are available on our website.
